Change by Action Project was founded by a registered nurse and law student who has worked at the intersection of health, education, and justice in Ghana.
While working at the bedside, our founder saw patterns that medical care alone could not solve. Preventable illnesses linked to poor health education. Young people limited by lack of access to information. Families affected not only by disease, but by weak systems and limited legal awareness.
These experiences shaped a clear conviction. Sustainable health outcomes require more than treatment. They require education, justice, and accountable systems.
To respond to this gap, Change by Action Project was established as a community-based organization focused on practical action aligned with the Sustainable Development Goals, particularly SDG 3 on health, SDG 4 on education, SDG 16 on justice and civic awareness, and SDG 17 on partnerships.
Our work combines grassroots health education, community engagement, and rights-based awareness to empower people with knowledge and tools they can use in their daily lives.
Change by Action Project exists to turn lived experience into structured action, and to ensure that care does not stop at the clinic, but extends into the systems that shape people’s futures.

Change by Action Project is built on collaboration. We seek project-based partnerships with local and international NGOs, UN agencies, corporate social responsibility teams, and Ghanaian institutions. These collaborations focus on designing, implementing, and scaling community programs in health, education, and civic awareness.
We welcome pro bono and technical support from lawyers, grant writers, monitoring and evaluation specialists, public health professionals, and communications experts. Professional contributions strengthen our internal systems and improve the quality and reach of our work.
Our founder and team are available for speaking engagements, panel discussions, interviews, and participation in leadership fellowships focused on global health, justice, education, and social entrepreneurship.
Strategic funding enables us to move from pilot activities to sustainable programs. We invite foundations, philanthropists, and sponsors aligned with SDGs 3, 4, 16, and 17 to engage with our project portfolio and impact priorities.
